Overview
LPA allows Singaporeans to appoint someone to make decisions on their behalf if they lose mental capacity.
Types of LPA
- Personal Welfare LPA: For decisions about personal care, healthcare, and living arrangements.
- Property & Affairs LPA: For financial and legal matters.
Application Process
- Must be 21 years and above, mentally capable.
- Complete the LPA form available from the Office of the Public Guardian (OPG).
- Both donor (person granting) and donee (appointed representative) must sign before a Commissioner for Oaths or a solicitor.
- Register the LPA with OPG.
Important Notes
- Registration required to be effective.
- Donor can revoke or amend the LPA anytime while mentally capable.
- OPG offers advisory services.
